Insurance Information

Updated 9/15/25: Duke Health has announced that all employee health plans will transition from Aetna to Cigna starting January 1, 2026. I will continue to be in-network with Aetna, but unfortunately, I will be considered out of network with Duke Basic and Duke Select plans as I am not in-network with Cigna. See below for more information about being an out-of-network provider.

🔶

As of 1/1/2025, I'm out of network (OON) with all insurances except Aetna (I do accept Duke Basic/Select through Aetna).

🔶

I'm happy to provide an itemized receipt (called a “super bill”) for you to submit to your insurance company and request potential reimbursement. See below for more detailed info.

🔶

What questions should I ask before starting services with Clancy Counseling PLLC, if I want to try to use my out-of-network benefits?

Call the number on the back of your health insurance card and ask for “member services.” The purpose of the call is to verify out-of-network coverage. In case you need to have this info, I am a Licensed Clinical Social Worker and my NPI number is 1225355399.

  • Do I have out-of-network coverage for mental health services provided through tele-health?

  • What is my yearly out-of-network deductible? Has it been met or how much more until my deductible is met?

  • How many sessions per year does my plan cover? (You can also ask if there is a lifetime maximum on mental health benefits)

  • How much does my insurance plan reimburse for an out-of-network provider for the following CPT codes?:

    • 90791 (intake/assessment)

    • 90837 (53+ minute session) *this is the code I use almost every time

    • 90834 (38-52 minute session)

  • What is my co-payment? (if applicable)

  • What is the home and mailing address on my file? (important to ensure reimbursement checks are issued to the correct address)

  • How do I submit a super bill? Is there an online portal or do I have to mail or fax a copy?

  • What is the time limit to submit a super bill?
